How to Pick a Dental SEO Agency (10 Things to Check First)

·

·

IN SHORT (In short)

Before you hire a dental SEO agency, check 10 things: dental-specific experience, HIPAA-compliant tracking with signed BAAs, public pricing, no long-term contracts, transparent monthly deliverables, real reporting, specialty-specific content, no fake guarantees, founder access, and a 30-day money-back option. Skip agencies that score under 7 out of 10.

Picking the right dental SEO agency in 2026 is harder than it should be. Most agencies look the same on the surface: nice website, vague promises about “more patients,” a tier list of monthly plans, and a sales call where someone pitches you services you do not fully understand.

RankingMedic was founded by Dr. Shahrooz Hassan, MBBS, who saw a gap in healthcare marketing specifically because I watched too many dentist friends pay agencies that did not understand HIPAA, did not run dental-specific playbooks, and did not actually move new patient numbers. Here is the 10-point checklist I would use if I were hiring an SEO agency for my own practice.

Do They Actually Specialize in Dental?

Short answer: A real dental SEO agency works with dentists, dental groups, and dental specialties as the majority of their clients. A generalist agency that does plumbers, law firms, and dentists is splitting attention and will not know the difference between an emergency caller and an insurance-shopping caller. Choose specialists.

Generalist agencies run the same SEO playbook for every industry. That works for plumbers. It fails for dentistry because dental search has unique patterns:

  • Emergency dental search behaves completely differently from cosmetic search
  • Insurance-driven patients convert faster but at lower margins
  • Implant and ortho cases have 10-30 day decision cycles
  • Pediatric and family dental searches lean heavily mobile

Ask the agency: what percent of their current clients are dental? If it is under 40 percent, they are a generalist using your account to learn.

Is Their Tracking HIPAA Compliant?

Short answer: Yes, this matters even though most dental practices do not realize it. Standard call tracking and Google Analytics 4 can collect protected health information (PHI) without a signed Business Associate Agreement. The HHS Office for Civil Rights has fined healthcare practices and their vendors for this exact gap. Always ask for a BAA.

The HHS Office for Civil Rights issued specific guidance in 2022 and 2024 making it clear that web tracking tools that handle PHI must operate under a signed Business Associate Agreement. Most marketing agencies do not have BAAs with their analytics vendors. That puts the legal risk on YOU, the practice owner.

Before signing, ask:

  • Do you have a BAA with your call tracking vendor?
  • Is your Google Analytics setup configured to scrub PHI?
  • If we get an OCR audit letter, can you produce a clean inventory of BAAs?

If they cannot answer these clearly, find an agency that does HIPAA-compliant marketing by default.

Do They Publish Their Pricing?

Short answer: Agencies that hide pricing usually charge different rates to different clients based on perceived budget. Public pricing is a sign of confidence and transparency. If you cannot find their fees on the website without a sales call, that is a yellow flag.

Public pricing tells you two things:

  1. The agency is confident enough in their value to put prices in writing
  2. You will not be charged more than another client just because you sound flush

It is fine for agencies to offer custom enterprise pricing for big multi-location deals. But the starting price for a typical single-location dental practice should be visible. Compare our pricing page to any other agency you are considering. The transparency itself is the signal.

Are There Long-Term Contracts?

Short answer: Avoid agencies that require 12 or 24-month contracts. Quality agencies offer month-to-month with 30 days notice to cancel. Long-term contracts protect the agency from being fired for bad work. They do not protect you.

The dental SEO industry standard for “good” agencies is month-to-month engagement. There are reasons why a few legitimate agencies require 3 or 6-month minimums (SEO is a compounding investment, leaving early can waste both sides’ time). But anyone asking for 12 or 24 months is hedging against being held accountable.

If the agency genuinely delivers value monthly, you will not want to leave. If they do not, you should be able to.

Are Monthly Deliverables Specific and Itemized?

Short answer: Yes, you should see a written list of what gets done each month: how many GBP posts, how many citations cleaned up, how many articles written, what gets reported. Vague deliverables like ‘ongoing SEO optimization’ usually means the agency does not have a real process.

Here is what a real monthly deliverable list looks like:

  • 4 GBP posts published
  • 2 procedure-specific landing pages built or updated
  • 5 to 10 citations cleaned up across healthcare directories
  • 1 round of internal link audit and updates
  • Monthly review responses to all new Google reviews within 48 hours
  • Monthly written performance report with rankings, traffic, calls, consultations

If the agency cannot put numbers like this in writing, they are improvising every month and you will get inconsistent results.

Do They Provide Real Reporting?

Short answer: Quality agencies provide monthly written reports showing keyword positions for your target queries, organic traffic trends, calls tracked from organic vs paid vs direct, consultations booked, and money-attributable revenue where possible. Skip agencies that send vanity metrics like ‘impressions’ or ‘reach.’

Real reporting answers one question: did the marketing dollars become booked patients?

You should see:

  • Keyword positions for 20 to 30 of your target queries (procedure + city searches)
  • Organic traffic month over month
  • Calls tracked with source attribution
  • Consultations booked tied back to source channel
  • Attributed revenue where the practice management system integrates

If your monthly report focuses on “impressions” or “reach” instead of these, you are getting vanity metrics. Real tracking and analytics measure outcomes, not activities.

Do They Promise Specific Rankings?

Short answer: No, and this is critical. Anyone who promises ‘first page rankings guaranteed’ is either lying or about to violate Google guidelines and tank your site. Google’s algorithm decides rankings, not the agency. What a quality agency can guarantee: monthly reporting, BAA-covered infrastructure, and a refund if quality is not delivered.

The “guaranteed rankings” pitch is one of the oldest red flags in SEO. Google explicitly warns against agencies that promise specific positions because the only way to guarantee them is through tactics that can get your site penalized or de-indexed.

What a quality agency WILL guarantee:

  • Specific monthly deliverables in writing
  • HIPAA-compliant infrastructure with signed BAAs
  • Direct founder or senior access (not layers of account managers)
  • 30-day money-back if quality is not what was promised
  • Asset ownership (you keep your GBP, content, and access on cancellation)

That is the legitimate set of guarantees. Anything more specific (like “we guarantee top 3 for these 10 keywords”) should make you walk away.

Founder Access Or Layers of Account Managers?

Short answer: Smaller agencies give you direct access to the founder or senior strategist. Bigger agencies route you through junior account managers who often do not understand the strategy. For dental practices that pay $1,500 to $5,000 a month, direct senior access is normal and reasonable.

Big agencies have account manager hierarchies because they need to scale. That works fine for enterprise clients. It does not work well for dental practice owners who need to talk to someone who actually understands dental SEO.

Ask: “Will I be working with the founder, a senior strategist, or a junior account manager?” If the answer is unclear, find an agency where the answer is clear.

Is There a Money-Back Guarantee?

Short answer: Quality agencies offer at least a 30-day money-back guarantee on the first month. If the work in month one is not what was promised, you get a refund. This is a sign that the agency is confident in their deliverables. Agencies that refuse this guarantee are protecting themselves.

The 30-day guarantee works because most SEO work in month one is foundation: audit, install tracking, set up GBP, fix on-page basics. If any of that is not done well, you should not be on the hook for the monthly fee.

Agencies that genuinely deliver are happy to offer this. Agencies that do shaky work avoid it. The presence of a clear, written guarantee is one of the best filters for finding quality.

Do They Specialize in Your Specific Niche?

Short answer: General dental SEO is fine for general practice, but specialty practices (oral surgery, orthodontics, pediatric, cosmetic, implants) need agencies with experience in that specific specialty. The keywords, conversion paths, and patient psychology differ enough that specialty experience matters.

Implant patients search differently than general dental patients. Pediatric searches lean heavily on parent psychology. Orthodontic searches are heavily influenced by Invisalign branding. Cosmetic searches involve longer consideration cycles and trust signals.

If you run a specialty practice, ask the agency for case studies or references from clients in your specialty. If they cannot produce them, they will be learning on your dime.

Frequently Asked Questions

How do I know if a dental SEO agency is legitimate?
Check three things: do they publish pricing publicly, do they offer month-to-month contracts with a money-back guarantee, and do they have HIPAA-compliant tracking with signed BAAs. Legitimate agencies pass all three. Agencies that fail one or more of these are usually hiding something.
What questions should I ask a dental SEO agency before signing?
Ask: What percent of your current clients are dental? Do you have a signed BAA with your call tracking vendor? Can I see your specific monthly deliverables in writing? Is there a money-back guarantee in the first 30 days? Who specifically will I be working with day-to-day? If they cannot answer all five clearly, find another agency.
Should I work with a local dental SEO agency or a remote one?
Location does not matter much for SEO. What matters is specialization and quality. A remote dental SEO specialist will outperform a local generalist almost every time. The work happens online anyway: GBP management, content writing, technical SEO. Choose the agency that knows dental, not the one that is in your zip code.
Can I do dental SEO myself?
Yes, technically. The basics (claiming your GBP, optimizing your site, getting on directories) are documented publicly. But most dental practice owners do not have 5 to 10 hours per week to do it consistently for 12+ months. Hiring a specialist usually pays back faster than DIY because of execution speed, not because the agency knows secrets you do not.
What is the worst thing a dental SEO agency can do?
The worst behaviors are buying spammy backlinks that get your site penalized by Google, using identical content across multiple client sites (duplicate content penalty), and operating analytics or call tracking without a BAA (HIPAA violation that puts your practice at legal risk). All three happen at low-cost agencies regularly. Always ask for written attestation that they do not do these things.
